Achieving that goal will require a significant reenvisioning of the diagnostic process and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. Diagnostic errors persist throughout all settings of care, involve common and rare diseases, and continue to harm an unacceptable number of patients. Beyond identifying diagnostic errors and near misses, organizational learning aimed at improving diagnostic performance and reducing diagnostic errors will also require a focus on understanding where in the diagnostic process the failures occur, the work system factors that contribute to their occurrence, what the outcomes were, and how these failures may be prevented or mitigated (see Chapter 3). Introduction of ethosuximide resulted in complete control of absence seizures in 47% (33 of 70) of the patients.
