The fluid can be examined with a light microscope, and crystals can be reliably visualized by experienced observers; however, easier and more definitive identification is done utilizing a compensated polarized light microscope (see Plate 5-15). A unique feature of the thoracic and upper lumbar levels of the spinal cord is small lateral horns of gray matter, which are the source of efferent sympathetic neurons of the autonomic nervous system. Some children can be fitted with a suction socket prosthesis as early as 5 years of age. Although these changes in bone geometry have been noted in all long bones studied, they are most striking in the femur. Small buds of tissue, representing the upper and lower limbs, first appear on the lateral body wall at about the 26th day. At this pseudoglandular developmental stage, the lung has a glandular appearance with many thinwalled tubes (arrows) and sacs (*) of various shapes and sizes lined by simple squamous to cuboidal epithelium. Diagnosis is not difficult after the illness has become chronic or when any of the following manifestations are present: rheumatoid nodules; rheumatoid factor in serum; characteristic joint deformities; and radiographic evidence of articular cartilage thinning, subchondral bone destruction, joint deformity, or ankylosis. Studies showed azathioprine use resulted in a lower requirement of prednisone, but response to therapy may take as long as 4 to 6 months. Multiple epiphyseal dyspla sia usually remains unrecognized until the child is 5 to 10 years of age. Clear cells primarily secrete water and electrolytes; dark cells elaborate macromolecular substances in sweat.
